Join NRG Dunkirk Power, LLC and the American Red Cross of Southwestern New York, together with other area business leaders and media personalities in recognizing those Chautauqua County residents who have acted above and beyond the call of duty. Be a part of this year’s spectacular Hero’s Celebration! Think about nominating neighbors, friends and professionals you know who have acted heroically. This year’s Hometown Heroes will be selected by a group of community leaders. Winners and their nominators will be recognized at a community breakfast celebration on May 18, 2012 at Moon Brook Country Club.

Please select the appropriate award category below, and complete the Nominee and Nominator information. On the reverse side of this form, use 100 words or less to describe why your nominee should be chosen for recognition at the Salute to Hometown Heroes 2012.

Nomination will be accepted for the following categories:

· Humanitarian - Sponsored by NRG Dunkirk Power, LLC: Any individual or group who has shown exceptional commitment to his/her community, who has made a positive impact in Chautauqua County

· Fire Fighter: Any individual or group, volunteer or paid, who acted heroically or has gone beyond the call of duty to make our community safer

· Law Enforcement: Any county, city or local town law enforcement member who acted heroically or has gone beyond the call of duty to make our community safer

· Emergency Personnel/Medical - Sponsored by Chautauqua Patrons Insurance Company: Any ambulance, emergency responder, 911 dispatch or medical professional who has acted heroically or gone beyond the call of duty to make our community safer

· Workplace - Sponsored by Cummins Engine: Any group or individual who has acted heroically on the job or any group or individual from a company that has made a positive impact on the community

· Youth - Sponsored by Key Bank: Any individual or group of youths who have made a positive impact on their community, or who performed a heroic act for another person in time of need

· Animal Rescue: Any animal who helped or saved a person in time of need, or person who helped or saved an animal
